- I am 18 years old or older.
- I agree and understand that is my sole responsability to perform a proper CLIENT CONSULTATION in order to not cause any injury or damage to my clients.
- I agree and I am fully aware that all the training offered by LASHSCIOUS are non-medical, however there are steps to be follow and regulations by HEALTH CANADA, which I will investigate and follow.
- I understand I may need insurance to practice any treatment or procedure offered on LASHSCIOUS ONLINE AND IN PERSON TRAINING.
- In the case that I am not satisfied with the training by LASHSCIOUS, I agree to immediately contact LASHSCIOUS regarding the issue. You also agree not to publicly defame LASHSCIOUS on any online or social media networks ie. Facebook, Twitter, Pinterest, Instagram, Google or Yelp prior to contacting us. Doing so is prohibited and we will seek legal action against your disputed claim. I understand that there are no refunds issued for any services or enhancements I received at LASHSCIOUS and I will contact LASHSCIOUS with any issues or concerns I may have.
- In consideration of (LASHSCIOUS) and Cecilia Andrea Morales Salas (Andrea Morales) by purchasing and completing any online or in person training I hereby forever releases and further agrees not to make any claim or demand or commence, loss or any relief whatsoever against (LASHSCIOUS) and/or Andrea Morales in respect of any cause, matter of thing whatsoever existing or relating to the procedures performed as described on the training courses.
- I agree that this release shall operate conclusively as an stoppels in the event of any such claim, action or proceeding and may be pleaded accordingly.
- This release shall be deemed to have been made and shall be constructed in accordance with the Laws of the Province of Ontario.
- This release shall ensure to the benefit of and be binding upon(LASHSCIOUS) the undersigned and their respective administrators, legal personal representatives, successors and assigns.
- By purchasing online or in person training, I agree that I have read and agreed to the POLICY and TERMS AND CONDITIONS posted by LASHSCIOUS on this website, and I understand and agree these can be modified at anytime, for which I WILL SIGN THE CONTRACT PROVIDED before starting my training, and send a copy to [email protected] if training is taken online.
- I am aware I have to sign the TRAINING PROGRAM AGREEMENT, and that failure to do so will mean I will be de-certified by LASHSCIOUS.
7. APPOINTMENTS
Deposit for appointments:
- Appointments require a deposit due to the amount of time booked in the schedule. These appointments require any cancelling or rescheduling to be done prior to 48 hours of your appointment, otherwise you will forfeit your deposit and must place another deposit when you rebook with us.
- If you book online a representative will contact you for a $25 CAD deposit, which will be taken off your total the day of your appointment.
- If you are going to be late to your appointment, please call us. If you are so late that we are unable to accommodate your appointment, you will be subject to the $25 late cancellation fee.
- Models are required a $25 deposit fee.
